Field Staff
|
Home Health Nurse (Registered Professional Nurse)
Qualifications :
* Registered Professional Nurse licenses in the State of Illinois
* Minimum of one year nursing experience in a hospital or home health/private duty agency
* Demonstrates good assessment skills with ability to work with minimal supervision
* Proof of ability to perform the essential job functions and is in good health as indicated in a current physician's statement of health status
Occupational Therapist
Qualifications :
* B.S. degree and graduation from an accredited school of Occupational therapy or is eligibkle for the National Registration Examination of the American Therapy Association; or, has two years of appropriate experience as an Occupational Therapist and has achieved a satisfactory rate on a proficiency examination conducted, approved or sponsored by the U.S. Public Health Service
* Current registration in the State of Illinois
* Work experience of at least one year under qualified Occupational Therapist, home health experience preferable
* Ability to make home visits and provide Occupational Therapy in patients' homes
* Communciation, interpersonal, and organizational skills necessary to complete job responsibilities
Physical Therapist
Qualifications :
* B.S. degree and graduate from a program approved by the Council of Medical Education of the American Physical Therapy Association or other comparable program
* Licenses as Physical Therapist in the State of Illinois
* Work experience of at least one year under a qualified Physical Therapist, home health experience preferred
* Ability to make home visits and provide physical therapy evaluations and treatments in the home environment.
All Field Staff
* Must have a current valid Illinois Driver's License and current required automobile insurance, if driving on agency business
* Must not pose a "direct threat" to the health or safety of the individual or others in the workplace or in performance of job
